CTAAE Incident Reporting

The purpose of this form is for students, faculty, staff, and others to report any type of safety event, concern, injury or similar occurrences that have happened with the facilities of the College of Technology, Architecture, and Engineering.This also includes issues such as inappropriate behavior.
Reports should be submitted as soon as possible.
For serious injuries or emergencies, please call 911 immediately.
